Fill in all three required information

1. Name of the web app or service and its website URL

Provide the full name and the website address of the service.

Example

"DocuSign — www.docusign.com"

2. Valid business justification

Explain clearly why this tool is needed or why existing tools cannot do the same job.

Example

"Our customer contracts require legally binding e-signatures. DocuSign is the platform our customers are already set up on and it provides an audit trail required for compliance. Microsoft's native signing does not meet this requirement."

3. Job function it supports and who will use it

State your role, the specific task it supports, and whether others on your team will also need access.

Example

"As Sales Coordinator, I prepare and countersign customer contracts daily. The Sales team of 3 people would need access."

4. What company data will be entered into the service

IT must assess any risk of company or customer data leaving the GETS environment. Be honest and specific.

Example

"Customer names, email addresses, and contract PDFs will be uploaded to the platform for signing."

Do not create an account or enter any company data into the service until IT has confirmed approval. Incomplete requests may be delayed or denied.

Click Submit and save the ticket number from your confirmation email. Use it to track status or follow up with IT.
